Here you’ll find all the information you need to make a referral for lifestyle support. We can help with healthy eating, becoming more active, stopping smoking, reducing alcohol intake and weight loss.
How do I refer?
Our secure online referral form is the easiest way to refer a patient to us, you will need an ID code to access this system, if you don’t know yours or you don’t have one already please contact us, otherwise please follow this link online referral form.
Paper referrals can be made as follows:
Health Trainer referrals - via the system in place at your practice/site which might be a book or self-reply cards patients fill in at reception. If you don’t have anything set-up then they can be made via phone.
For specialist physical activity support (Exercise Referral) select the correct form from those listed below:
Who can I refer?
Children aged between 8 and 11.
Anyone aged 16 and over.
