Apply for LinCare services
You can use this service to apply for LinCare Services or apply on behalf of someone else.
LinCare services allow individuals to stay in their homes for longer and independently by providing a safe and monitored environment.
You can also use this service to apply on behalf of someone else.
Before you start
To use this service you will need:
- Address and postcode
- Date of birth
- Medical details
- Contact details for one or both of the following:
- two individuals that are either next of kin or keyholders
- or Wellbeing Lincs Response Service (this costs £2.50 week to be signed up to)
You can access LinCare services by completing the online form, contacting the LinCare Control Centre on 0300 303 4430 or emailing LinCare@lincoln.gov.uk where we can send out an application pack.
